Community Living Connections (CLC) is seeking passionate individuals to join our team as Supported Living Staff , providing essential, person-centered support to individuals with developmental disabilities. Whether you're an experienced caregiver or new to the field, we will provide the training you need to succeed!

Location: Madison West Side

PT w/Two Shift Options Available:

  • 12 PM to 8 PM -or-
  • 3 PM to 5 PM
  • Rotating Weekends

Key Responsibilities:

  • Person-Centered Support : Implement individualized care plans tailored to the unique needs and goals of each person you assist. This includes fostering personal growth, life skills, and community integration.
  • Daily Living Assistance : Provide support with household tasks such as cooking, cleaning, and organizing, while encouraging independence in these activities.
  • Health & Wellness : Help with personal care routines like hygiene, grooming, and promoting healthy living habits, ensuring a safe and clean environment.
  • Community Engagement : Support individuals in exploring and utilizing community resources, attending appointments, and participating in social activities.
  • Financial & Transportation Support : Help with budgeting, grocery shopping, and transportation needs based on each individual's schedule.
  • Communication & Collaboration : Partner with families, managers, and external professionals to provide high-quality care.

What We're Looking For:

  • A desire to support individuals with developmental disabilities.
  • Patience, empathy, and excellent communication skills.
  • Ability to work independently and as part of a team.
  • Willingness to participate in ongoing training and professional development.
  • Flexibility to work varying shifts, including weekends and evenings.
  • A valid WI driver's license, access to a reliable vehicle, and proof of vehicle insurance that meets CLC's standards.

Qualifications:

  • High School Diploma or equivalent.
  • No prior experience necessary-we provide all required training.
  • Ability to maintain a valid driver's license and meet driving record standards.

Physical Requirements:

  • Ability to lift up to 40 lbs. occasionally.
  • Ability to stand, walk, bend, and assist individuals with daily activities.
